Year of Registration - Clan Wars*
Applicants should nominate the year they wish to register their team to play Clan wars, ie 2017 or 2018 and the season, Season 1 or 2 or both. Please see the Clan Wars page for more details.
Season 1 & 2 Registration - Clan Wars
There are two seasons each year for both State and B-League, Season 1 and Season 2. Applicants can select the season they wish to register, either Season 1 or 2 or the full year. Please see the Clan Wars page for more details.

This refers to your team name only and not individual members name. This is requested later in this form.

Please list each team member's full name, Call Sign (Nicknames) and contact number here.
